The Buffalo Medical Group is required by federal and New York State law to protect medical
information about you and to provide you with a notice about our privacy practices and
obligations. Your medical records and x-rays are strictly confidential, and are the property of
the Buffalo Medical Group. Your medical information cannot be released to anyone without
written permission. However, it is important to note that if you are a member of a health
maintenance organization (HMO), your subscriber contract gives the HMO the authority to
conduct a professional review of your medical record.
Upon your initial registration at a BMG office, you will be offered a copy of our privacy
practices which explains how your health information may be used during your course of
treatment, and that we are required by law to maintain the confidentiality of any health
information that identifies you. In addition, you will be asked to sign a statement indicating
that you have received the privacy practices notice. We ask that you take a few minutes to
review this important information. If you have any questions about the BMG’s privacy practices,
please don’t hesitate to talk with your doctor or a member of our
support staff.
As a Buffalo Medical Group patient, you are entitled to receive
quality medical care and have the right to:
- Considerate and respectful care
- Privacy concerning your medical care
- Expect that all communications and records pertaining to your
care will be treated as confidential
- Obtain from your physician complete, current information
concerning your diagnosis, treatment and
prognosis in language you can easily understand
- Receive from your physician information necessary
to give informed consent prior to the start of any
procedure and/or treatment
- Refuse treatment to the extent permitted
by law and to be informed of the medical
consequences of your action
- Expect reasonable continuity of care
- Examine and receive an explanation of your
bill regardless of source of payment
